How to Report

The filing of a formal, written complaint initiates the formal grievance process and is available to any person who is participating in (or attempting to participate in) a university educational program or activity. The Title IX coordinator (or an investigator designated by the Title IX coordinator) will investigate the allegations in the formal complaint.

A person may submit a formal complaint in writing to the UAFS Title IX Coordinator. A complaint form is available in the Title IX Office (located in Room 230 of the Smith-Pendergraft Campus Center), or you may submit your own. To be considered a formal complaint under UAFS Title IX procedures, the submission must meet the following criteria:

Recommended Elements of a Formal Complaint

  • It must be a written document (typed or clearly handwritten).
  • It must contain your physical or digital signature.
  • It must allege sexual harassment or other sexual misconduct against an individual and request that the University investigate the allegation(s).
  • It must be written in your own words and may not be authored by others, including family members, advisors or attorneys.
  • It should identify the person alleged to have committed the misconduct (if known).
  • It should describe the incident(s) with reasonable specificity, including:
    • What occurred
    • When it occurred (date or approximate timeframe)
    • Where it occurred (location or context)
  • It should include a list of any relevant sources of information such as:
    • Witnesses
    • Records or correspondence
    • Photos, messages, or documentation

Note: Under UAFS Title IX policy, at the time of filing your formal complaint, you must be participating in or attempting to participate in a UAFS education program or activity.

Please note: UAFS Staff will assist a reporter in making a formal report to the police if that assistance is desired.

If you want full confidentiality, essentially to talk through what happened but not officially report it, and the issue is not an immediate life emergency, speak with a professional counselor, clergy member, or health care provider (e.g., the UAFS Student Counseling Center, 479-788-7752 or the Crisis Intervention Center Hotline at 1-800-359-0056). The Title IX coordinator will assist with this if you wish.
