Change of Information

Please note that all UAFS employees must go to Human Resources for address changes, and residency changes will require documentation.

To request a change of address and/or telephone number, please complete and turn in a Student Address Change (PDF).

You may also request a change by e-mail through LionsLink at records.department@uafs.edu. (Please note that requests for changes can only be accepted from a LionsLink address.)

Your request must include a copy of court-ordered name change or a Social Security Card with the new name.
