Release Forms

All Cub Camp attendees must complete the following Release Forms. You may type on the attached forms. Please print the completed forms and fax, mail or submit in person.

All forms must be submitted prior to camper paticipation.  Students submitting all release forms by July 26, 2012 will receive a $25 credit.

PHYSICAL LOCATION

The Cub Camp Office is located on the UA Fort Smith campus in the Smith-Pendergraft Campus Center in Room 108.

RELEASE FORMS

The following three (3) Release Forms must be submitted in their entirety in order to attend Camp. Those campers who submit all three (3) Release Forms by July 26 will receive a $25 credit.
